All forms below are to be used by current students and require your TTC credentials to access. Once you click on any of the forms listed below you will be directed to enter your username and password to access the form.

Address/Phone Number Update – Students can update their address and phone number through the User Profile in Self-Service.

Change of Major Request – Students can use this form to request a change to their program of study. You will receive notification via your my.tridenttech.edu email once your student record has been updated.

Cross Registration Form – This form should be completed when a TTC or CSU student intends to cross register for the Fall or Spring semester under the “Tuition-Free Policy.” This form will serve as the application, authorization, and registration form.

Duplicate Diploma Request - To order a duplicate or replacement diploma, complete this form. The cost of each replacement diploma is $10. Payment can be paid to the Business Office. For questions related to this form, email graduation@tridenttech.edu.

Enrollment Verification Request – Students can download or print their Enrollment Verification Certificate from the TTC Portal. Click here for information and directions.

Graduation Application – Students in their final semester will complete this application through Self-Service. Students are encouraged to confirm all their program requirements will be met with their advisor prior. Click here for more information about graduation eligibility and the ceremony.

Release of Information – Students can use this form to designate who has access to their student information. You will receive notification via your my.tridenttech.edu email once your student record has been updated.

Social Security Number Change – Students can use this form and provide documentation to correct their social security number on their student record. You will receive notification via your my.tridenttech.edu email once your student record has been updated.

Student Name Change – Students can use this form and provide documentation to change the name on their student record. You will receive notification via your my.tridenttech.edu email once your student record has been updated.

Transcript Request – Current students, former students, and alumni can request an electronic or printed Trident Technical College transcript. Click here for information and directions.

Transient Approval – This form is to be used by a current student who is in good academic standing and wants to take a course at another regionally accredited institution for transfer credit back to TTC. This form should be completed by the student with the assistance of their academic advisor. Final approval must be received by the dean before submitting to the Registrar's office.
