Affidavit of Support, Form I-134 This form should be used by sponsors of prospective students to show ability to provide sufficient funds to students for educational and living expenses throughout the length of study. This form needs to be notarized by a certified notary public. F-1 Transfer Clearance Form This form should be used by prospective students wishing to transfer to TTC from another school. The form must be filled out and signed by the current school’s DSO and mailed/faxed directly to us. F-1 Transient Form This form should be used by prospective students enrolled at another college but wishing to take summer courses at TTC. The form must be filled out and signed by the current school’s DSO and mailed/faxed directly to us. F-1 Tuition Deposit Refund Request Form This form should be used by current F-1 students that have completed two semesters at TTC and wish to have their tuition deposit refunded to them or applied towards the tuition fees of the following semesters. This form must be picked up in the International Students office. F-1 Reduced Course Load Request Form This form should be used by current F-1 students that wish to request a reduced course load. The form must be filled out and signed by the student’s academic advisor and mailed/faxed directly to us by the advisor. F-1 Completion of Program Verification Form This form should be used by current F-1 students that need to indicate their anticipated graduation date in order to extend their I-20 or to complete their program of study. The form must be filled out and signed by the student’s academic advisor and mailed/faxed directly to us by the advisor. F-1 Waiver for Change of Status Form This form should be used by current F-1 students that have applied for change of immigration/nonimmigrant classification or permanent residency.
