ACADEMIC / FINANCIAL AID SUSPENSION APPEAL REQUEST
(Spring Semester 2009)
 
 
  The deadline to appeal is 5:00 PM, January 6, 2009, the day of open registration as indicated in OnCourse. The appeal request must be completed before the deadline in order to be considered. Please read the following instructions for the appeal process. By forwarding to the next page, you are acknowledging that you have read and understand the following guidelines to the appeals process.  
 
 
 
1)    You must type, in the space provided, a detailed explanation of the extenuating or unusual circumstances, which caused your poor academic performance or excessive withdrawals.
 
2)    You must submit written documentation of the extenuating / unusual circumstances, i.e., medical reports, death certificates, police reports, to the Information Counter in the 410 Building. This documentation must show dates corresponding to all affected semesters and must be included even if you have appealed before.
 
3)    Provide information and documentation explaining how the extenuating / unusual circumstances have been resolved.
 
4)    The committee will consider all information provided. Additionally, the committee will review your entire academic history.
 
5)    You must turn in the appeal as soon as possible after being made aware of your suspension. The committee may meet after midterm to consider the appeals received by that time (only for students who are not enrolled in the present semester.)

    Approval of an academic appeal does not include reinstatement into a curriculum program. Approval is only for re-admission to the College.
 
    Financial aid appeals are not retroactive. You will not receive money for a previous semester.
 
    The Committee's decision is final.
