Ann Hosford Smith Phlebotomy Scholarship Fund

Established in 2005 by colleagues and friends of Ann Hosford Smith who served as department head for Diagnostic and Imaging Services and as coordinator for the Health Sciences certificate in Phlebotomy; the Fund assists students enrolled in the Phebotomy program  who demonstrate financial need.   

Eligibility Criteria: To be considered for this scholarship, an applicant:

1)      must be a returning or continuing student;

2)      must be enrolled in the Phlebotomy certificate program;

4)      must enroll in and maintain at least six credit hours;

5)      must have completed at least 10 credit hours (including AHS 105, AHS 142, AHS 143 with a grade of 'C' (2.0) of greater);

6)      must have a cumulative GPA of 3.0 or greater; and

7)      must demonstrate financial need (determined from completed FAFSA form);

Essay Requirement:  Applicants are required to provide a brief essay that explains their professional goals and financial need.  Instructions for submitting the essay will be provided during the application process.

Award amount/use: Award of up to $100 for tuition, books, required course materials and equipment.
