Application for ADN Program Merit Placement

I am submitting my application for Merit Placement into the Fall 2009 ADN class.
I understand that the purpose of Merit Placement is to provide students already admitted to the ADN nursing program the opportunity to be considered for an earlier start date.  I further understand that points are awarded on published criteria, which I have reviewed, and that students are ranked based on total number of points.  Students with the highest number of earned points will be selected to move their start date forward as space becomes available. I understand that if I am not selected for the semester listed above, I will retain my original space in the nursing program and I may apply for Merit Placement in the future.
I qualify to be considered for Merit Placement because I have: (Check all that apply) ______ qualifying language skills scores ( Circle one: critical reading SAT or ENGL ACT              scores or ENG 101 grades) ______ qualifying analytical and reasoning skills scores ( Circle one: Math SAT or                Math ACT scores or MAT 110/120 grades) ______ qualifying sciences (Circle one: average of BIO 210 and 211 grades; average of              two high school biology of chemistry courses) ______ a bachelor’s degree or higher with a minimum cumulative GPA of 3.0 ______ an associate in science degree with a minimum cumulative GPA of 3.3 ______ high school rank within the top 10% of graduating class ______ letters of recommendation (high school applicants only)
Proof that I have met the criteria above is attached and includes: (Check all that apply) ______copy of qualifying SAT or ACT scores ______official or student copies of all of my college transcripts, including TTC.           ______copy of my high school transcript ______two (2) letters of recommendation (high school applicants only)
I understand that if my application as submitted is determined by TTC to be incomplete my application will not be considered for Merit Placement for the semester listed above.
I certify that I can complete immunizations for Hepatitis B, rubella, rubeola, tetanus and varicella by August 26, 2009.  I understand that if I do not submit proof that  these immunizations have been completed by the last day of drop/add for FastForward I,  I will not be permitted to register for nursing courses and will no longer be eligible for consideration for Merit Placement.  If this occurs, I must request to defer to the next available space in the nursing program with no further option for Merit Placement.
I have met the March 3, 2009 deadline for submitting this Application for Merit Placement into the Fall 2009 class. I understand that applications received before February 25 or after March 3, 2009 at 1 pm will be removed from consideration for Merit Placement with no further option to apply.
